Job Hours: 40 hrs/wk: M & F 8a-5p, Tu & W 10a-7p, Th 9a-6p
Salary: Bilingual CHN 1: $22.30 - $30.93 / Hour; Bilingual CHN 2: $24.02 - $33.30 / Hour

Summary of Essential Duties:

Telephone Triage, patient education. Assess and manage walk-in patients. Responsible for Medication Refill by Protocol. Responsible for Lab & Imaging review, routing. Teamwork in delivering quality care.

Education:

CHN1 – Associates degree in Nursing from an accredited college or university is required. Bachelor’s degree in Nursing preferred. College-level coursework in the social sciences, and additional nursing training within a community or public health curriculum desirable.

CHN2 – Bachelor of Science in Nursing from an accredited college or university.

 

Experience:

CHN1 – Nursing experience in public or community health care services desirable.

CHN2 – Two years of responsible population-focused community or public health nursing experience.

 

Substitution: An equivalent combination of experience and training that will demonstrate the required knowledge and abilities is qualifying.

Bilingual abilities (English/Spanish) required. Lane County will administer bilingual testing as a part of the selection process.

 

Special Requirements: CHN1 & CHN2

Possession of a current State of Oregon Registered Nurse license at time of appointment. Possession of a valid Oregon driver’s license and CPR Certification at time of appointment.

 

Knowledge of:

Practices, principles, procedures and theory of public and community health nursing. Community resources available for public and community health nursing problems. Policies and protocols of relevant program and clinic operations.  Examination, assessment and treatment techniques.

 

Ability to:

CHN1 – Screen and assess levels of wellness.  Work with a team nursing approach, and monitor and assess medical conditions. Communicate clearly and concisely, both orally and in writing. Establish, maintain and update a variety of reports, charts and documentation associated with public and community health care.  Maintain confidentiality of communications.

CHN2 – The abilities listed above and to independently assess, identify and treat illnesses and diseases associated with public and community health nursing services following medical standing orders and acting within nursing scope and standards of practice. Identify problems and generate solutions within the scope of the department, division, program or clinic.
